WHAT AGES DO YOU SEE?

We are a child and youth clinic – therefore we cannot see persons over the age of 18. If you are turning 18 while connected to our clinic, we will discuss planning around transitioning you to adult resources and back to your primary care provider. For psychiatric services, Summit Health and Soulshine Health provide adult outpatient psychiatric services.

HOW DO I GET REFERRED?

Our clinic requires referrals from your primary care provider (GP, NP) – please discuss your concerns with them and if a referral would be helpful or not in your unique situation. If you do not have a primary care provider, we accept referrals from schools, counselors, and through the hospital.

WHICH DOCTOR WILL I SEE?

Our team works in a collaborative model, meaning that we work together to help support all the families we see. Our pediatricians rotate when they provide care, meaning that you may see a different doctor depending on who is available for when you need follow-up.

For our psychiatric team, you will be booked with whoever can see you first for your initial appointment – once you are connected with a psychiatrist, you will see that same person for follow-ups.

DO YOU PROVIDE ON-GOING FOLLOW-UP?

We tailor our follow-ups depending on you and your family’s unique situation – in some situations, this may be short-term follow-up with discharge back to your primary care provider, in other instances it may be longer-term follow-up.

WHY DO MY APPOINTMENTS GET RESCHEDULED?

Our physicians are on-call for the hospital providing emergency care – there may be unexpected changes to your appointment as a result, thank you for your understanding around this, we know it can be difficult.

CAN YOU HELP CONNECT US WITH LOCAL RESOURCES?

We are glad to support navigating and liaising with local services and agencies that may be helpful in your care (some examples may be: schools and the Department of Education, counselors/therapists, Child and Youth Advocate Office, CYFN, YFNED, Family and Children’s Services, Disability Services).

WHAT IF I MOVE OUT OF THE TERRITORY?

We cannot continue to provide care if you move out of the territory – we will help support you to find local resources.
